Introduction of Pouchitis

Pouchitis, a nonspecific, idiopathic inflammation of the ileal reservoir, is the most common long-term complication following in patients with an ileal-pouch anal anastomosis (IPAA) for ulcerative colitis (UC). Clinically, pouchitis is characterized by variable symptoms, including increased stool frequency, fecal urgency, abdominal cramping, and bleeding, with occasional fever and extraintestinal inflammation. Possible factors that contribute to pouchitis are an increased bacterial concentration and an altered composition of the microflora in addition to a history of UC. Given this association, antibiotics and probiotics have been used to treat patients with pouchitis. Recent studies have shown that probiotic treatment is effective in maintaining remission in pouchitis.

Treatment of Pouchitis with Probiotics

Recently, there has been an increasing number of reports that probiotic therapy is effective for inflammatory bowel disease, including pouchitis, and might be an effective approach to improve the quality of life in ulcerative colitis patients. In vitro and animal studies have shown that probiotics influence epithelial barrier function and gut permeability, restore the commensal bacterial microbiota, and reduce the production of inflammatory cytokines. In recent years, a series of placebo-controlled randomized clinical trials have been conducted demonstrating the clinical efficacy of probiotics and the effectiveness in the maintenance of remission in pouchitis patients. In addition to maintaining remission, probiotics have been shown to decrease relapses of chronic pouchitis.

The highly concentrated probiotic mixture VSL#3 is effective in the prevention of pouchitis onset and in maintaining antibiotic-induced remission. VSL#3 contains 300 billion bacteria per gram of viable lyophilized bacteria of 8 different probiotic strains. The study has demonstrated that in patients with recurrent or refractory pouchitis who have achieved remission with intense antibiotic treatment, the probiotic therapy VSL#3 is highly effective in maintaining remission in preventing pouchitis onset, and significantly improving the quality of life of patients.
